FollowMyHealth Patient Portal: Personal Health Record

At Knox Community Hospital we believe that every patient should have easy, instant access to his or her health information at any time. For the convenience of our patients, KCH now offers one Patient Portal for both provider records and hospital records. These services are available to those with valid e-mail addresses. This opportunity is available through the use of a secure internet portal.


The Patient Portal is compatible with Chrome, Firefox, Safari, and Microsoft Edge.

Knox Community Hospital believes that every patient should have easy, instant access to their health information at all times. KCH offers the Follow My Health Patient Portal for ease of access to your records and additional services from the Department of Primary and Specialty Care. All that is needed is a valid email address.

All patient information related to Knox Community Hospital will be uploaded to the Follow My Health Portal as well once linked.
This portal can be accessed from any computer, tablet, or smartphone, and allows you to:

  • Access results
  • Message your provider
  • Refill a prescription
  • View upcoming appointments
  • Request, cancel, or reschedule appointments
  • Access your clinical summaries from your Physician office visit(s)

What is the FollowMyHealth® portal?

Knox Community Hospital's FollowMyHealth Patient Portal offers convenient access to your medical records, prescription refills, messaging your provider, requesting, canceling, rescheduling, or viewing upcoming appointments. All that is needed is a valid email address.

How much does it cost?

The FollowMyHealth® portal is a free service offered by Knox Community Hospital to our patients. 

Is my medical information and records safe and secure?

Yes! Knox Community Hospital takes great pride in ensuring all health information is kept private and secure. Access is controlled through a secure invitation process, personal ID’s and passwords.

My spouse and I both would like to create an account. Should we use one email for both of us?

We recommend using separate emails for each adult in the family who is creating a Follow My Health portal. This will ensure only that members can see results, and messages. If you would like to utilize the same email, please contact 740.393.9051 and select option 1 to properly have your accounts linked to both. Each user will need their own username and will want to keep in mind any updates for the accounts will go to the listed email address.

What if I forget my username or password?

You can reset your username and password using the links on the Follow My Health log-in screen. You can also call our Medical Records specialist team at 740.393.9051 option 1.

Why am I unable to view all of my medical information?

Your portals may not be linked, contact 740.393.9051 and select option 1 to get your portals connected.

How do I download the app to my phone?

You can download the free app by searching “FMH” on your iPhone, iPad, or Android from the App Store on your device.

Ways to Look Up Your Records

You can look up your patient records for Knox Community Hospital and Physician Practices by downloading the Follow My Health App on your smartphone or tablet or by going online to the link below.
