How Does It Work?

Once your provider prescribes home infusion therapy, our team will help with the coordination and planning of your care. 

  1. Before you leave the hospital, a member of our team coordinates with you and your doctor to make sure your therapy and supplies are delivered to your home.
  2. Upon discharge from the hospital, a nurse may visit you in your hospital room or one may be scheduled to visit you in the home.
  3. Throughout your care, our pharmacy team works with you to coordinate refills and deliveries to make sure your therapy is always there on time.
  4. Our team works with you to create a plan of care based on the goals to meet your therapy needs.
  5. Your provider routinely receives an update on your progress to ensure the best results for your therapy.

Our nursing staff works closely with our pharmacy team to make sure your home infusion therapy is administered safely and effectively. 

Our team regularly updates your provider about your progress and provides support and resources to you and your caregiver so you can comfortably receive care in your home.
