Obtain my medical records:

1. FOR MAIL-IN ACCESS
   You may complete an AD-66 (form to access medical records) by printing the authorization and mailing the completed form.

   We will send your records immediately upon request.
   Fees and postage may apply.
    

2. If you are visiting our office, the authorization form is available in the Medical Records department, you can download or print and complete it before you visit. In most cases, your request can be provided to you in a very short waiting period or we can mail it to whoever you request.
    
3. You may access our patient portal (Follow My Health) to view and manage your personal health information.  To set up an account please follow the steps outlined here:www.kch.org/patient-portal.
