JOB SUMMARY

  Ensures all services and education provided are evidence-based. Provides disease management education to patients in individual, group and community settings based on the needs of the patient population. Serves as a consultant/resource to physicians/providers, staff members, and the community. Collaborates and communicates effectively with team members, physicians, and other healthcare providers to coordinate the medical plan of care. Develops, implements and evaluates outreach education programs and health screenings for the community. Adheres to organizational policies and procedures; regulatory accrediting body requirements; and professional practice standards.

Primary Responsibilities:

  • Identifies, develops, implements and evaluates evidence-based disease management education programs and resources for patients, staff members, and to serve the community.
  • Develops and implements individualized dietary care plans in accordance with the physician’s orders. Utilizes nutritional assessment methodology to include planning individualized menus, nutrient calculations and modifications of diets.
  • Assesses/reassesses patients’ disease management educational needs based upon observation of patient and interview of patient and/or family, physician’s diagnosis and review of data, diagnostic results, and other relevant information from other healthcare providers.
  • Provides patients and family members (as applicable) with evidence-based health care education and disease management instructions.
  • Facilitates and conducts support groups as applicable to disease management.
  • Serves as a consultant; collaborates and communicates effectively with team members, physicians, and other healthcare providers to coordinate the medical plan of care.
  • Documents all aspects of patient care/education and maintains patient records and care plans in a timely manner.
  • Responds to patient referrals/consults in a timely manner.
  • Serves as a resource to the community; develops, implements and evaluates outreach education programs and health screenings for the community.
  • Assists to develop evidence-based disease management protocols for patient care.
  • Assists to monitor data and implement/participate in performance improvement initiatives for disease management, national and/or state regulatory requirements/standards and quality and safety patient initiatives.
Requirements Include
  • Bachelor’s Degree in Nutrition/Dietetics or related field required
  •   Current registration status with the Commission of Dietetic Registration
  • Current licensure to practice Dietetics in the State of Ohio
  • Minimum of 3 years’ experience
  • Teaching experience in a patient care, classroom and/or community setting preferred
  • Certification as a Diabetes Educator required or completed within designated timeframe
