Community Care Network (CCN) for Our Patients

The Community Care Network is a free service that provides support to reduce your risk of hospital readmission.

Health concerns that may result in referral include:

  • Congestive Heart Failure
  • COPD
  • Diabetes
  • Hypertension
  • Chronic Pain
  • Pneumonia

The Community Care Network program is a 15 week program that provides social support, one on one interactive attention to your health with education and training to help you learn to manage chronic concerns before they result in a hospital readmission.

The following professionals will assist with your care:

  • Licensed Dietitian
  • KCH Medical Director, Dr. Ron Martinson
  • Registered Nurse
  • Licensed Social Worker
  • Community Care Network Trained Health Coach

Your health coach will visit once a week to assess nutrition habits, medications, need for assistance getting to doctor appointments and to provide ongoing education.

Our trained health coaches are college age students pursuing a career in healthcare. Coaches must apply for the opportunity to become a health coach and meet the criteria of the program.
