CCN Health Coach

The Community Care Network of Knox Community Hospital is a collaborative partnership between KCH and local colleges and universities.

The program offers students pursuing degrees in the Health Sciences field to provide weekly visits to patients with chronic or ongoing conditions, and to provide support and assistance in meeting their health goals.

Students must apply to be accepted into this unique program and will be trained as Health Coaches and assigned to patients enrolled in the Community Care Network (CCN). Patients are enrolled following a referral and interview process as well as a comprehensive health assessment including a plan of care. KCH has established an interdisciplinary team to meet and develop the plan of care that addresses each patient’s health needs and goals.

The program is free to enrolled patients.

Student Requirements:

Students wishing to participate in the CCN must meet the following requirements:

  • Complete the application process
  • Provide 2 letters of recommendation
  • Have and maintain a GPA of 3.25 or higher
  • Attend a 15-week, 1.5-hour orientation/lecture
  • Commit to approximately 4 hours per week and lasting a minimum of 12 weeks including weekly patient visits
