Patient Access - Registration/Scheduling

We will verify personal (name, date of birth, etc.) and insurance information.

During your registration process, you will be asked to show all insurance cards and sign a consent form. We are required to ask you about Advance Directives and religious preference. If you are a Medicare patient, you will also need to complete a Medicare Secondary Payer questionnaire. The average registration takes six minutes.
