Respirators must be used in workplaces in which employees are exposed to hazardous airborne contaminants.
When respiratory protection is required employers must have a respirator protection program as specified in OSHA’s Respiratory Protection standard (29 CFR 1910.134).
Before wearing a respirator, workers must first be medically evaluated using the mandatory medical questionnaire or an equivalent method. To facilitate these medical evaluations/regulatory requirements, Knox Community Hospital and Mid-Ohio Corporate Care (MOCC) has developed a simple and secure on-line OSHA respirator questionnaire.
With this service, an employer or safety manager can simply provide his/her staff with a URL to an electronic version of the questionnaire and then have each employee complete the questionnaire and submit it electronically. These questionnaires are, then, promptly reviewed by the medical director of Mid-Ohio Corporate Care. The employer can choose to receive individual clearance documents or a summary. Either way, MOCC maintains electronic documentation should you ever be audited.
