RN - Home Health - Oncall, PRN

This  RN position Provides direct care and instructs clients and caregivers in prevention and care of health concerns.  Comprehensively assesses clients; re-evaluates clients as needed.  Develops and implements individualized care plan for each patient’s needs and revises plan as needed.  Prepares documentation. Collaborates needed changes and coordinates goals of all disciplines (such as physicians, therapists, etc).   Establishes appropriate client goals among all disciplines promoting independence through education of patient and caregivers.  Demonstrates critical thinking to assure the delivery of safe, quality, age specific care.   Recognizes developmental and biopsychosocial changes in patients and caregivers with an understanding of Holistic Nursing.  Case management of all disciplines from initial evaluation  through discharge and assists in establishing appropriate community resources.  Adheres to organizational policies and procedures; regulatory accrediting body requirements; and professional practice standards, scope of practice.

The on-call registered nurse is responsible for, providing skilled nursing, intervention, and supportive care to patients in the home setting for evening, night, weekend, and holiday on-call duty in accordance with Knox Community Hospital Department of Home Health Plan of Care for each patient.

The on-call RN will be on a rotating weekly evening/nightly on-call schedule and rotating weekend call (Friday 8am – Monday 8am). Weekday after-hours shifts are Monday through Friday 4:30pm until 8am the following morning.

On-Call RNs occasionally may also be offered PRN shifts during regular business hours (Monday- Friday, 8am – 4:30p). Final on-call hours/days can be negotiable.

 

Requirements Include: 
  • Graduate of an accredited program for nursing education, BSN preferred.
  • Current licensure to practice professional nursing in the State of Ohio.
  • Current American Heart Association (AHA) Healthcare Provider CPR (BLS) certification required.
  • Valid Ohio’s Driver License and maintain motor vehicle insurance
